In the past week, I’ve been to three newly “reopened” medical offices for appointments, and all of them screened me with a COVID-19 questionnaire and temperature checks.
None of them asked me my age nor any questions about any health risks such as diabetes, obesity, or the like.

It seems that the question of how old you are and do you have any risk factors would be a no-brainer to be added to these questionnaires.
